Though I'm taking the social mores of the setting into account, ie, "Would others in this society think that this person has a problem?" In the case of Alec, they do.
ETA: I am surprised no one has suggested that I diagnose Thomas Covenant. (Someone else can do it; I only got as far as the rape in book one.)
